Joined up Services

outcome 5Shropshire’s Health & Wellbeing Board commits to developing better joined up services and better access to services and information through integration. The Board views integration is the means by which continuity of care is delivered across time and care settings; focusing on delivering services designed around individual needs. Joining up services (or integration of services) won’t look the same for each service area, but will have flexibility to develop as needed and with the input from service users, professionals and carers.
